Which formats are there?

Non-blacklisting affidavit, Bid Security Declaration (in place of EMD), authorization letter to sign and submit the bid, undertaking to accept all tender terms, declaration of no near relative in the department, and Make in India local content self-declaration.

Do I need stamp paper?

For an affidavit, yes – print it on non-judicial stamp paper of the value asked in the tender (often ₹100) and get it attested by a Notary. Declarations and letters are usually printed on your firm's letterhead.

Can I change the wording?
